About HealthierHere

HealthierHere is transforming the way health care is delivered to thousands of residents in King County. We work across medical, behavioral health, social service, and community to build a welcoming, accessible, and integrated delivery system that fosters health and wellness for all.

We partner with:

Behavioral Health Agencies

Behavioral health is integral to an individual’s overall health and well-being. Behavioral health agencies provide services critical to addressing the needs of the whole person. We partner with behavioral health agencies as critical partners in the effort to integrate physical and behavioral health services, and to address the needs of community members with mental illness and substance use disorder. We are also committed to improving opioid prescribing practices and helping Washington state achieve its Integrated Managed Care goals. 

Community-based Organizations (CBOs) and Social Services

Community-based organizations and social service agencies provide critical supports to ensure that individuals stay healthy in their communities, or can transition back to their communities after an illness. We work with trusted community partners that provide an array of services to underserved populations, from culturally and linguistically competent systems navigation and advocacy to housing, transportation, food assistance, and more.

Community Health Centers

Community Health Centers (CHCs) provide a variety of critical health-related services to diverse and underserved people in King County, regardless of their ability to pay. We partner with CHCs to support their work reaching underserved community members, delivering whole person care, and addressing community health needs using a team-based approach.

Hospitals and Health Systems

Hospitals and health systems are key partners to help King County achieve the quadruple aim: better health, better quality of care, lower costs, and improved equity. We work with hospitals and health systems seeking to reduce the number of unnecessary emergency department visits and hospitalizations, improve patient transitions back to the community, and reach community members in need of whole person care. 

Community Members

HealthierHere’s work is ultimately about transforming the delivery system so that community members receive higher quality care and are at the center of decision-making about their care. Our partners are taking a patient-centered approach to systems transformation and recognizing what makes our communities unique. What are we working on?

At the heart of our work is an understanding that many factors affect our health, and a belief that everyone should have the opportunity to live a healthy life. Our programs focus on moving beyond the clinic and hospital walls by addressing a whole person’s needs, building strong partnerships between healthcare, behavioral health and social service systems, and implementing large-scale improvements in our delivery system that reduce health disparities.
