About the presenter:
An experienced social worker, Wei-Lin Huang has served in both inpatient and outpatient settings. One of her most recent roles was as a behavioral health specialist in an integrated primary care clinic. Wei-Lin’s process improvement experience includes assisting practices with work flows and team-based care.
The MeHAF Assessment is a valuable tool that can be used to drive organizational progress toward implementing patient- and family-centered integrated care. HealthierHere requires clinical practice partners to complete the MeHAF every 6 months.

About the MeHAF:
The MeHAF Assessment is a nationally recognized self-assessment tool that assesses a clinical site’s progress toward implementing patient- and family-centered integrated care. The assessment focuses on two domains:
- Integrated services and patient and family services, and
- Practice/organization.
The Washington State Health Care Authority (HCA) adopted the 21 question MeHAF+ as a required pay-for-reporting requirement for all Accountable Communities of Health (ACHs). All ACHs are required to have participating sites complete the survey every six months and submit the results to the HCA.
HealthierHere requires all of our contracted clinical practice partners to complete the MeHAF every six months, in line with the HCA. We see a lot of value in having clinical care teams complete the MeHAF and use it as a tool to promote broad-based conversations among care team members about their progress on implementing integrated care.
